Domiciliary care
Medication support at home
Prompts, reminders and agreed assistance so medicines are taken as prescribed — with clear records at every visit.

Overview
About medication support
Medicines are one of the most common reasons families worry when someone is living alone. Our medication support is arranged at a level that matches what the person actually needs, from a simple reminder to agreed assistance recorded at each visit.
What we can do is set out clearly in the care plan and is reviewed if a prescription changes. We do not take on clinical tasks that fall outside the scope of a home care service.
What it covers
What this support can include
- Prompting and reminders at agreed times
- Assistance taking medicines from a dispenser or blister pack
- Support with creams and topical preparations where agreed
- Recording what was taken, refused or missed at each visit
- Flagging low stock so prescriptions can be reordered in time
- Raising concerns promptly with the agreed contact
Who it suits
Who this is often right for
- People managing several prescriptions across the day
- People living with memory difficulties who find timings hard
- People recently discharged with a changed medicines list
- Families who cannot be there at every dose time
How we work
Our approach
Written into the care plan
The level of support, the medicines involved and the timings are all agreed and recorded before support begins.
Recorded every visit
Staff complete a medication record at each visit so there is a clear, checkable history.
Clear limits
Where a task needs clinical input, we say so and support the family to speak to the GP or district nursing team.
