Domiciliary care
Hospital-to-home support
Practical help in the first days and weeks back at home, while confidence and routines return after a hospital stay.

Overview
About hospital-to-home
The move home from hospital is often the point where things become difficult. The house has been empty, the fridge is bare, the medicines have changed and everything takes longer than it did before.
Hospital-to-home support is designed for that period. It can be intensive at first and reduce as someone finds their feet again, or continue as ongoing care if that is what is needed.
What it covers
What this support can include
- Preparing the home for a return — heating, food and essentials
- Personal care while strength and confidence rebuild
- Medication prompts around a changed prescription
- Meals, drinks and encouragement with eating
- Support moving safely around the home
- Laundry, tidying and everyday household tasks
Who it suits
Who this is often right for
- People being discharged after surgery, illness or a fall
- People whose needs have changed while in hospital
- Families arranging support at short notice around a discharge date
- People who need more help at first and less over time
How we work
Our approach
Flexible from the start
We expect the plan to change in the first fortnight and review it as recovery progresses.
Clear about scope
We provide non-clinical support. Where nursing or therapy input is required, we will say so.
Joined-up communication
We keep the agreed family contact informed about how things are going.
